Prominent veins after 65: when should you worry?

Visible veins aren’t automatically a warning sign of a dangerous condition, even after age 65. As skin becomes thinner and loses some elasticity with age, veins can naturally become more noticeable. Low body fat, exercise, heat, and genetics can also make veins prominent.

However, new or suddenly more prominent veins accompanied by other symptoms deserve attention.

Possible causes

  • Varicose veins or chronic venous insufficiency: Veins may become enlarged or twisted, often with aching, heaviness, itching, or ankle swelling.

  • Superficial thrombophlebitis: A vein can become painful, red, warm, and firm.

  • A blood clot (DVT): More typically causes new swelling, pain, warmth, or redness in one leg, rather than simply visible veins.

  • Heart or other circulation problems: Leg swelling and breathlessness alongside prominent veins warrant medical assessment.

A newly swollen or painful leg is accompanied by sudden shortness of breath, chest pain, fainting, or coughing blood. These can be signs of a blood clot that has traveled to the lungs.

Also seek medical advice for rapidly worsening leg swelling, skin discoloration, ulcers, or persistent pain.

Bottom line: Prominent veins by themselves are often a normal consequence of aging and body composition. It’s the sudden change and accompanying symptoms, rather than the visibility of the veins alone, that matter.
