7 medications that cause severe dementia and profound brain damage

There isn’t a scientifically accurate list of 7 medications that inevitably cause “severe dementia and profound brain damage.” Most medications associated with cognitive problems cause temporary confusion/delirium or increased dementia risk, rather than directly causing irreversible brain damage.

Medications/classes with particularly important cognitive risks include:

Diphenhydramine (Benadryl) — strongly anticholinergic; can cause confusion and delirium, especially in older adults.
Oxybutynin — an anticholinergic bladder medication; long-term exposure has been associated with increased dementia risk.
Amitriptyline — tricyclic antidepressant with substantial anticholinergic effects.
Benzodiazepines such as alprazolam, lorazepam, and clonazepam — can cause sedation, memory impairment and delirium; prolonged use is associated with cognitive concerns.
