7 Medications That May Affect Memory and Cognitive Health
Some medications can cause confusion, drowsiness, or memory problems, particularly in older adults. Long-term exposure to certain medicines has also been associated with a higher risk of dementia.
That doesn’t mean these medications inevitably cause dementia, or that you should stop taking them. Many are useful and sometimes necessary treatments.
1. Diphenhydramine
Diphenhydramine is a first-generation antihistamine found in some allergy and sleep medicines. It has strong anticholinergic effects, which can cause confusion and other cognitive side effects, especially in older adults.
2. Amitriptyline
This older antidepressant has significant anticholinergic activity. It can cause dry mouth, constipation, drowsiness, and cognitive problems in some people.
3. Paroxetine
Paroxetine is an antidepressant with relatively strong anticholinergic properties compared with many other antidepressants.
A healthcare professional may consider alternatives when cognitive side effects or anticholinergic burden are concerns.
4. Oxybutynin
Oxybutynin is commonly prescribed for overactive bladder. Because it has anticholinergic effects, it can contribute to confusion and memory problems, particularly in susceptible older adults.
5. Certain Tricyclic Antidepressants
Medications such as doxepin at higher doses, imipramine, and nortriptyline can have anticholinergic effects.
Long-term exposure to strong anticholinergic medications as a group has been associated with increased dementia risk in observational studies.
6. Some Antipsychotic Medications
Certain antipsychotics have anticholinergic properties and can cause sedation or cognitive side effects.
They can nevertheless be appropriate for specific medical conditions, so the risks and benefits need to be considered individually.
7. Benzodiazepines
Medicines such as alprazolam, lorazepam, and diazepam can cause drowsiness, impaired memory, confusion, and problems with coordination.
The FDA warns about dependence, withdrawal, misuse, and potentially serious interactions with other medicines or substances. They should not be stopped suddenly after regular use because withdrawal can be dangerous.

Not necessarily.
Research has found an association between higher cumulative exposure to strong anticholinergic medications and dementia, but observational research cannot prove that every medication in this category directly causes dementia in every person.
Short-term confusion or memory problems can also be reversible medication side effects, particularly after starting a drug, increasing its dose, or combining several medicines.
