17 Signs of Possible Kidney Trouble You Shouldn’t Ignore

Kidney disease can be silent in its early stages, so symptoms alone can’t tell you whether your kidneys are healthy. However, some changes deserve medical attention.

  1. Foamy or persistently bubbly urine — may indicate protein leaking into the urine.

  2. Blood in the urine — pink, red, or cola-colored urine should be evaluated.

  3. Puffy or swollen eyes — particularly persistent swelling around the eyes.

  4. Swollen ankles or feet — can occur when the body retains fluid.

  5. Swelling of the hands or face.

  6. Urinating more frequently, especially at night.

  7. A noticeable reduction in urine output.

  8. Persistent changes in urine appearance, such as unusual cloudiness.

  9. Unexplained fatigue or weakness.

  10. Difficulty concentrating or feeling mentally foggy.

  11. Persistent itching.

  12. Muscle cramps or weakness.

  13. Loss of appetite.

  14. Nausea or vomiting.

  15. A metallic or unusual taste in the mouth.

  16. Shortness of breath, particularly alongside swelling.

  17. High blood pressure, which can both contribute to kidney disease and occur as kidney function declines.

What about #3?

Puffy eyes do not automatically mean kidney disease. Allergies, lack of sleep, aging, and many other conditions can cause them. Kidney-related fluid retention is more concerning when swelling is persistent or occurs elsewhere, such as the feet and ankles.

How to actually check your kidneys

The most useful screening tests aren’t visual—they include:

  • Serum creatinine and eGFR to assess kidney filtration.

  • Urine albumin-to-creatinine ratio (uACR) to detect protein leakage.

If you have blood in your urine, very little urine, rapidly worsening swelling, severe breathing difficulty, or sudden confusion, seek medical care promptly.
