The skin around my ankles is getting darker and thicker over time. What causes this?

Darkening and thickening skin around the ankles is often related to chronic venous insufficiency—when blood has difficulty returning from the legs to the heart. Blood and fluid can pool around the lower legs, causing swelling, itching, and gradual brown or reddish skin discoloration.

Other possibilities include:

  • Stasis dermatitis from long-term venous congestion
  • Varicose veins
  • Previous leg swelling or a blood clot
  • Skin inflammation or eczema
  • Certain infections or other skin conditions
  • Less commonly, systemic conditions that affect skin pigmentation

When to get it checked

Because the change is progressively darkening and thickening, it’s worth having a clinician examine it rather than treating it as simple pigmentation. They may look for swelling, varicose veins, pulses, and signs of venous disease.

Seek urgent care if one leg suddenly becomes swollen, painful, warm, or red, or if you develop chest pain or shortness of breath.
