A month before a heart attack, your feet warn you of these six signs.

Your feet can sometimes reveal clues about your circulation and overall health—but there is no reliable evidence that your feet consistently warn you of a heart attack exactly one month in advance.

Some changes in your feet and ankles can be associated with heart or blood vessel problems, and knowing which signs matter can help you seek care sooner.

1. Swollen feet or ankles

Fluid buildup can occur with heart failure, particularly when swelling affects both legs. It can also result from kidney problems, medications, or prolonged sitting.

2. Pain in the feet or calves when walking

Cramping that repeatedly occurs during walking and improves with rest may indicate peripheral artery disease, which is associated with an increased risk of cardiovascular disease.

3. Unusually cold feet

Persistently cold feet may be associated with poor circulation, although cold temperatures and other conditions are common explanations too.

4. Blue, purple, or unusually pale toes

These color changes may indicate reduced blood flow or low oxygen levels. Sudden discoloration, especially with severe pain or a cold foot, requires emergency assessment.

5. Swelling that leaves an indentation

If pressing a swollen ankle briefly leaves a dent, fluid may be accumulating in the tissues. Persistent or worsening swelling deserves medical evaluation.

6. Foot swelling accompanied by breathlessness or unusual fatigue

This combination can indicate heart failure or another serious condition. Seek prompt medical assessment, especially if symptoms are new or worsening.

The most important warning

Foot symptoms alone cannot reliably predict a heart attack a month ahead. The American Heart Association identifies these more established warning signs:

  • Chest pressure, squeezing, or discomfort.

  • Pain spreading to the arms, back, neck, or jaw.

  • Shortness of breath.

  • Cold sweats, nausea, or sudden lightheadedness.

If these symptoms are happening now, call your local emergency service immediately. Do not wait for foot symptoms to appear or pass.
