12 Things to Be Careful About While Taking Amlodipine

Amlodipine has relatively few food restrictions, so “avoid these 12 things” is a little misleading. The bigger issue is medication interactions, excessive alcohol, and anything that makes your blood pressure drop too far.

1. Don’t stop amlodipine suddenly

Unless your clinician tells you to, continue taking it as prescribed. Stopping treatment can allow your blood pressure to rise again.

2. Be cautious with alcohol 🍷

Alcohol can add to amlodipine’s blood-pressure-lowering effect, potentially making dizziness or fainting more likely.

3. Don’t routinely drink large amounts of grapefruit juice

Grapefruit may increase amlodipine exposure in some people. The interaction isn’t as predictable or strong as with some other calcium-channel blockers, but caution is reasonable.

4. Don’t take sildenafil or similar medicines without discussing them

Amlodipine can contribute to low blood pressure when combined with sildenafil. Your prescriber should know about the combination.

5. Tell your doctor about antibiotics you’re taking

Some antibiotics—particularly strong CYP3A inhibitors such as clarithromycin—can increase amlodipine concentrations and may require monitoring or dose adjustment.

6. Be careful with antifungal medicines

Medicines such as itraconazole and ketoconazole can affect amlodipine metabolism and potentially increase its effects.

7. Don’t assume every blood-pressure medicine is safe to combine

Amlodipine is sometimes intentionally prescribed with other blood-pressure drugs, but combinations can lower blood pressure too much. Your prescriber should coordinate the doses.

8. Pay attention if you’re taking simvastatin

This is an important interaction: amlodipine increases simvastatin exposure. U.S. prescribing information recommends limiting simvastatin to 20 mg daily when used with amlodipine.

9. Don’t start herbal supplements without checking

Some supplements can affect drug metabolism. In particular, St. John’s wort can affect CYP3A4-related drug metabolism, so tell your pharmacist or doctor before using it.

10. Don’t ignore persistent ankle swelling 🦶

Swollen ankles are a well-known amlodipine side effect. Don’t automatically start a “water pill” yourself; your clinician can determine whether the dose should be adjusted or another treatment considered.

11. Be cautious with anything that makes you dizzy

When getting out of bed or standing up, do it gradually if you feel lightheaded. Sit or lie down if you become dizzy.

12. Don’t ignore severe symptoms

Seek urgent medical attention for fainting, severe dizziness, difficulty breathing, swelling of the face/lips/tongue, or new/worsening chest pain.
