Magnesium + Blood Pressure or Heart Medicines: What Should You Avoid?
The headline “magnesium clashes badly with blood-pressure medicines” is too broad. Magnesium supplements can interact with some medicines, but they don’t generally make common blood-pressure drugs stop working. The important interactions depend on the specific medication.
Medicines worth paying particular attention to
1. Diuretics (“water pills”)
Examples include furosemide (Lasix), bumetanide, hydrochlorothiazide, and other thiazide or loop diuretics. These medicines can change how much magnesium is lost through urine. Potassium-sparing diuretics such as spironolactone and amiloride have a different effect and can reduce magnesium loss.
2. Certain antibiotics
Magnesium can bind to doxycycline, ciprofloxacin, levofloxacin, and other tetracycline/quinolone antibiotics, reducing their absorption. This isn’t primarily a blood-pressure interaction, but it’s an important one. NIH advises taking these antibiotics at least 2 hours before or 4–6 hours after magnesium.
3. Bisphosphonates for osteoporosis
Magnesium can reduce absorption of medicines such as alendronate. They should be separated by at least 2 hours.
What about common heart medicines?
There isn’t a general rule saying you must avoid magnesium if you take amlodipine, metoprolol, lisinopril, losartan, or similar blood-pressure medicines. Magnesium itself can lower blood pressure slightly, so combining a supplement with antihypertensive treatment could potentially contribute to low blood pressure in some people, particularly with high supplemental doses.
⚠️ The bigger concern: too much magnesium
For adults, the NIH upper limit for magnesium from supplements and medications is 350 mg/day unless a healthcare professional recommends otherwise. Magnesium naturally present in food isn’t included in this limit. Excessive supplemental magnesium can cause diarrhea and, at very high levels, low blood pressure, abnormal heart rhythms, breathing difficulty, and potentially cardiac arrest. The risk is much greater when kidney function is impaired.
