Sarcopenia: Causes of Muscle Loss and Solutions

Sarcopenia is the age-related loss of muscle mass, strength, and physical function. It becomes more common with increasing age, but inactivity, poor nutrition, illness, and certain medications can accelerate muscle loss.

Main causes

  • Aging: Muscle protein synthesis becomes less responsive to food and exercise, and hormonal changes can contribute.
  • Physical inactivity: Bed rest, sedentary living, or prolonged hospitalization can rapidly reduce muscle mass and strength.
  • Insufficient protein or calories: Not eating enough protein, overall food, or essential nutrients makes it harder to maintain muscle.
  • Chronic diseases: Conditions such as diabetes, kidney disease, heart failure, cancer, and chronic lung disease can promote muscle loss.
  • Inflammation: Persistent inflammation can increase muscle breakdown.
  • Hormonal changes: Declining testosterone, estrogen, growth hormone, and other age-related changes may contribute.
  • Vitamin D deficiency: Low vitamin D is associated with poorer muscle function, although supplementation is most useful when deficiency is present.
  • Certain medications and illnesses: Some treatments, including prolonged corticosteroid use, may contribute to muscle wasting.

What helps prevent or reverse it?

1. Resistance training — the most important intervention
Strength training 2–3 times per week can stimulate muscle growth and improve strength. Exercises can include squats or sit-to-stands, rows, presses, lunges, and resistance-band exercises. Older adults should start gradually and increase resistance as they become stronger.

2. Get enough protein
For many older adults, approximately 1.0–1.2 g of protein/kg body weight/day is a reasonable target, with higher amounts sometimes appropriate during illness or rehabilitation. Spread protein across meals rather than consuming most of it at once.

Good sources include eggs, milk/yogurt, fish, chicken, lean meat, beans, lentils, soy, and other legumes.

3. Eat enough overall calories
Even a high-protein diet may not prevent muscle loss if you’re consistently under-eating. Adequate calories, carbohydrates, healthy fats, fruits, and vegetables support training and recovery.

4. Stay physically active
Walking, climbing stairs, balance exercises, and other daily movement complement resistance training and help preserve independence.

5. Correct deficiencies and treat underlying disease
A clinician can assess for problems such as vitamin D deficiency, thyroid disorders, inadequate nutrition, or chronic disease when muscle loss is unexplained or progressing rapidly.

6. Prioritize sleep and recovery
Adequate sleep and recovery between demanding workouts support muscle repair and physical performance.

A simple weekly approach

  • 2–3 days: Full-body resistance training
  • Most days: Walking or other moderate activity
  • Each meal: Include a meaningful protein source
  • Daily: Adequate fluids and a balanced diet
  • Regularly: Monitor strength, weight, walking ability, and ability to perform everyday activities

Important: Sarcopenia isn’t simply “getting thin with age.” Loss of strength and function is particularly important. If someone is experiencing unexplained or rapid muscle loss, repeated falls, difficulty rising from a chair, or significant weakness, medical assessment is appropriate.
