Sarcopenia: Causes of Muscle Loss and What Actually Helps
Sarcopenia is the progressive loss of muscle strength and, often, muscle mass and physical performance. It becomes more common with aging, but it isn’t an inevitable part of getting older—and muscle can often be rebuilt or preserved.
Why do we lose muscle?
Common contributors include:
- Aging and hormonal changes
- Too little physical activity, especially prolonged sitting
- Not eating enough protein or calories
- Illness or hospitalization and periods of bed rest
- Chronic diseases such as cancer, COPD, heart failure and kidney disease
- Poor nutrition and unintended weight loss
- Inflammation and other age-related changes in muscle metabolism.
🏋️ The most effective solution: resistance exercise
The strongest recommendation for treating sarcopenia is progressive resistance training. This can include weights, resistance bands, machines, or appropriately challenging body-weight exercises. It can improve strength, muscle mass and physical function.
For someone starting out, exercises such as:
- Sit-to-stand from a sturdy chair
- Wall or counter push-ups
- Resistance-band rows
- Step-ups
- Light squats
can be useful, with the difficulty gradually increased as strength improves.
If someone is frail, has significant weakness, or has balance problems, getting an individualized program from a physical therapist or qualified exercise professional is safer than starting an aggressive routine.
🍳 Protein matters too
Adequate protein is important, particularly in older adults. Some clinical guidelines suggest roughly 1.0–1.5 g of protein per kg of body weight per day for people with sarcopenia, although the appropriate amount depends on the individual.
Good sources include:
- Eggs
- Fish
- Chicken and other lean meats
- Greek yogurt and cottage cheese
- Milk
- Beans and lentils
- Tofu and other soy foods
It’s generally better to obtain protein from a balanced diet rather than assuming a supplement is necessary.
Important: People with significant kidney disease may need a different protein target, so don’t dramatically increase protein intake without discussing it with a clinician.
☀️ What about vitamin D or supplements?
Vitamin D is important for bone and muscle health when someone is deficient, but vitamin D by itself isn’t an established treatment for sarcopenia. Evidence for many muscle-building supplements remains limited.
🔎 How do you know if you have sarcopenia?
Difficulty rising from a chair, climbing stairs, carrying groceries, walking normally, or repeated falls can be warning signs. Clinicians can assess strength with tests such as grip strength or chair-stand testing and, when appropriate, measure muscle mass with methods such as DXA.
