Piriformis Stretch for Back, Hip, Glute and Leg Pain

A piriformis stretch may help some people with buttock or hip discomfort, particularly when the piriformis muscle is tight. But the idea that one “deep stretch” will eliminate all back, hip, glute and leg pain is misleading. Those symptoms can come from several different conditions, including lumbar nerve compression, arthritis, tendon problems or other causes.

🧘 A gentle piriformis stretch

Try this only if it doesn’t increase your symptoms:

  1. Lie on your back with both knees bent.
  2. Place your right ankle over your left thigh, forming a figure four.
  3. Gently bring the left thigh toward your chest.
  4. You should feel a stretch in the right buttock—not sharp pain or electrical pain down the leg.
  5. Hold for about 20–30 seconds, then relax.
  6. Repeat 2–3 times on each side.

Don’t force the knee toward the floor or aggressively pull the leg. A stretch should feel like tension, not pain.

⚠️ When stretching may not be the answer

Pain traveling from the lower back through the buttock and down the leg can be caused by sciatica or another irritated spinal nerve. In that situation, the piriformis may not be the underlying problem, and repeatedly doing a deep stretch could aggravate symptoms.

Seek medical assessment if you have persistent or worsening pain, significant weakness or numbness, or difficulty walking.

Get urgent medical help if back/leg pain is accompanied by new loss of bladder or bowel control, numbness around the genitals or anus, or severe/progressive weakness in both legs.

Bottom line: A gentle piriformis stretch can be useful for some people, but there is no single stretch that reliably eliminates back and leg pain. The right exercise depends on what’s actually causing the pain.
