The biggest misconception is that high cholesterol usually causes noticeable symptoms. It usually doesn’t. Most people feel completely normal, and the only reliable way to know your cholesterol is high is a blood test (lipid profile). (NHLBI, NIH)

In cases of very high cholesterol, particularly inherited familial hypercholesterolemia, certain physical signs can sometimes appear:

  1. 🟡 Yellow patches around the eyelids (xanthelasma)
    Cholesterol-rich deposits can form around the eyes.
  2. 🟡 Yellowish bumps on the skin (xanthomas)
    These can occur around the elbows, knees, hands, ankles or other areas. (NHLBI, NIH)
  3. 👁️ A gray-white ring around the cornea (corneal arcus)
    This becomes more common with age, so in an older adult it doesn’t necessarily mean high cholesterol. In a younger person, it can be worth investigating.
  4. 🦵 Leg pain when walking
    High cholesterol can contribute to plaque buildup in arteries. If peripheral artery disease develops, it can cause aching or cramping in the legs during walking that improves with rest. This is a symptom of artery disease—not high cholesterol itself.
  5. ❤️ Chest pain or shortness of breath
    These can occur when cholesterol-related plaque contributes to coronary artery disease. Again, these are symptoms of the resulting cardiovascular disease, not a direct symptom of high cholesterol.
