There is an important misconception in claims like this: most people with fatty liver disease have no obvious symptoms at all. Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly called nonalcoholic fatty liver disease (NAFLD), is often discovered through blood tests or imaging performed for another reason. 

The “hidden sign” may not be a symptom

One of the most important clues is actually a cluster of metabolic risk factors, rather than something you can necessarily see or feel.

People with MASLD commonly have one or more of the following:

  • Excess weight, particularly around the waist
  • Insulin resistance or type 2 diabetes
  • High triglycerides or abnormal cholesterol
  • High blood pressure
  • Metabolic syndrome 

Having these conditions doesn’t prove that you have fatty liver, but they increase the likelihood.

If symptoms occur

Fatty liver may cause:

  • Persistent tiredness
  • A general feeling of being unwell
  • Discomfort in the upper-right abdomen 

These symptoms are nonspecific, so they cannot diagnose fatty liver by themselves.

How is fatty liver actually detected?

Doctors may use liver blood tests, imaging such as ultrasound, and other tests to determine whether fat is present and whether there may be liver inflammation or scarring. 

When to take symptoms seriously

Yellowing of the eyes or skin, abdominal swelling, significant leg swelling, unusual bruising, confusion, or other signs of advanced liver disease should receive medical attention. These symptoms can indicate significant liver damage rather than simple fatty liver. 

Bottom line: There isn’t one hidden symptom that everyone with fatty liver has. The condition is often silent, which is why metabolic risk factors and appropriate medical testing are much more useful than trying to identify a single visible sign.
