Sarcopenia: Causes of Muscle Loss and What Actually Helps
Sarcopenia is the age-associated loss of skeletal-muscle strength and function, often accompanied by reduced muscle mass. It becomes more common with aging, but it isn’t an inevitable part of getting older—and muscle strength can improve at almost any age.
Why does muscle loss happen?
Several factors can contribute:
- Aging: muscles become less responsive to the signals that stimulate muscle protein production.
- Physical inactivity: prolonged sitting, hospitalization, or bed rest can accelerate muscle loss.
- Too little protein or overall food intake: inadequate nutrition makes maintaining muscle harder.
- Chronic illnesses: conditions that cause inflammation or reduced activity can contribute.
- Hormonal and metabolic changes associated with aging.
- Weight loss without strength training: losing weight can include loss of muscle as well as fat.
🏋️ The most important solution: resistance exercise
Progressive strength training is considered first-line treatment for sarcopenia. Exercises can include weights, resistance bands, machines, or appropriately challenging body-weight movements. Evidence shows resistance training can improve strength and physical function in older adults with sarcopenia.
Examples include:
- Sit-to-stand exercises
- Wall or countertop push-ups
- Resistance-band rows
- Step-ups
- Light weights or resistance machines
Start at an appropriate level and increase resistance gradually. If someone is frail, has significant weakness, or has medical conditions affecting exercise safety, a physiotherapist or qualified exercise professional can tailor the program.
🍳 Protein matters too
Older adults with sarcopenia may benefit from a protein-rich diet, particularly when combined with resistance exercise. International guidance commonly considers around 1.0–1.5 g of protein per kg of body weight per day in people with sarcopenia, although the appropriate amount depends on health, kidney function, diet and individual circumstances.
Good sources include eggs, fish, chicken, yogurt, milk, beans, lentils, tofu, and lean meats.
You don’t necessarily need expensive protein powders if you can obtain adequate protein from food.
🧪 What about vitamin D?
Vitamin D is important for bone and muscle health, particularly when someone is deficient, but vitamin D alone isn’t an established treatment for sarcopenia.
🚩 When should muscle loss be evaluated?
Unintentional muscle loss, increasing difficulty climbing stairs, repeatedly falling, struggling to rise from a chair, or noticeably declining walking ability deserves medical evaluation. A clinician can assess strength, physical performance and muscle mass and look for underlying causes.
