10 important side effects of metoprolol

  1. Tiredness or fatigue — One of the more commonly reported effects.

  2. Dizziness or lightheadedness — Particularly if blood pressure becomes too low.

  3. Slow heartbeat (bradycardia) — Metoprolol deliberately slows the heart rate, but an excessively slow rate can cause problems.

  4. Low blood pressure — May cause weakness, dizziness, or fainting.

  5. Shortness of breath or wheezing — More concerning in people with asthma or other bronchospastic conditions.

  6. Cold hands and feet — Reduced peripheral circulation can sometimes produce cold extremities.

  7. Depression or mood changes — Depression has been reported with metoprolol.

  8. Sleep disturbances — Headaches, nightmares, and insomnia have been reported.

  9. Gastrointestinal symptoms — Diarrhea, nausea, constipation, or stomach discomfort can occur.

  10. Worsening heart failure symptoms — In susceptible patients, metoprolol can sometimes worsen heart failure, particularly when therapy is initiated or adjusted. DailyMed+1

Two particularly important precautions

Don’t stop metoprolol suddenly. Abrupt discontinuation can worsen heart-related problems, including myocardial ischemia. It should generally be reduced gradually under medical supervision. DailyMed

If you have diabetes, metoprolol can also mask some warning symptoms of low blood sugar, such as a rapid heartbeat. DailyMed

Seek urgent medical attention for fainting, severe difficulty breathing, severe or worsening chest pain, or a very slow/irregular heartbeat.

The exact risks also depend on whether you’re taking metoprolol tartrate or metoprolol succinate, your dose, and your other medications.
