Sarcopenia: Causes of Muscle Loss and Solutions
Sarcopenia is a condition involving loss of skeletal-muscle strength and, usually, muscle mass. It becomes more common with aging, but it is not simply an unavoidable part of getting older and can also occur earlier because of illness, inactivity, inadequate nutrition, or prolonged immobility.
Common causes and risk factors
- Aging: Muscle mass and strength tend to decline over time.
- Physical inactivity: Long periods of sitting, bed rest, or reduced activity accelerate muscle loss.
- Insufficient protein or overall nutrition: Inadequate food intake can make it harder to maintain muscle.
- Chronic diseases: Conditions such as cancer, heart failure, COPD, and kidney disease can contribute to secondary sarcopenia.
- Hospitalization or prolonged recovery: Reduced movement can cause substantial muscle loss, particularly in older adults.
- Obesity: A person can have excess body fat while simultaneously having inadequate muscle strength or mass (“sarcopenic obesity”).
Signs worth noticing
Difficulty rising from a chair, climbing stairs, carrying groceries, walking at your usual speed, or maintaining balance can indicate declining muscle function. Weak grip strength is another useful warning sign. Formal assessment may include grip strength, a chair-stand or walking test, and measurement of muscle mass when appropriate.
What actually helps?
1. Progressive resistance exercise
This is the most strongly supported intervention. Exercises such as appropriately prescribed squats or chair stands, resistance bands, and weight training can improve strength and physical function. Guidelines strongly recommend resistance-based exercise for sarcopenia.
2. Adequate protein and calories
A protein-rich, balanced diet can support muscle maintenance, particularly when combined with resistance exercise. Protein recommendations should be individualized, especially for people with kidney disease or other medical conditions.
3. Stay physically active throughout the day
Walking and other aerobic activity are valuable for overall health, but they shouldn’t completely replace resistance training when the goal is maintaining muscle strength.
4. Treat underlying problems
If muscle loss is significant or happening unexpectedly, clinicians may look for contributing conditions, nutritional deficiencies, medication effects, or prolonged inactivity. Treating an underlying disease is an important part of managing secondary sarcopenia.
5. Don’t rely on supplements alone
There is currently no established medication that universally reverses sarcopenia. Exercise and nutrition remain the foundation of treatment.
