Magnesium Supplements: Medications That May Interact

The claim “never use magnesium if you’re taking these medications” is too absolute. Magnesium can interact with some medicines, but in many cases the solution is to separate the doses by several hours, adjust the dose, or have a clinician/pharmacist monitor you.

Important examples include:

  1. Certain antibiotics — magnesium can bind to tetracycline antibiotics (such as doxycycline) and quinolone antibiotics (such as ciprofloxacin), reducing their absorption. They generally need to be taken at different times.

  2. Bisphosphonates — medicines such as alendronate can have reduced absorption when taken with magnesium. Separate administration is generally recommended.

  3. Diuretics — some “water pills,” including loop and thiazide diuretics, can affect magnesium levels. The interaction isn’t simply a reason to avoid magnesium, but monitoring may be appropriate.

  4. Proton-pump inhibitors (PPIs) — long-term use of medicines such as omeprazole can lower magnesium levels in some people. A healthcare professional may monitor magnesium rather than automatically advising against supplementation.

  5. Some thyroid medicines — magnesium-containing products can interfere with absorption of levothyroxine if taken too close together. Your pharmacist can advise on appropriate spacing.

One particularly important precaution

People with significantly reduced kidney function should be especially cautious with magnesium supplements. The kidneys normally remove excess magnesium, so impaired kidney function can allow magnesium to accumulate.
