The skin around my ankles is getting darker and thicker over time. What causes this?

Darkening and thickening skin around the ankles can have several causes, and the pattern you describe is worth getting checked rather than treating it as simply a cosmetic change.

One common cause is chronic venous insufficiency. When blood pools in the lower legs, pressure in the veins can cause inflammation and leakage of blood pigments into the skin. Over time, the skin may become brownish, thickened, itchy, or hardened, often around the ankles.

Other possibilities include:

  • Eczema or chronic skin inflammation, especially if there is itching or scaling.
  • Repeated friction or scratching, which can gradually thicken and darken skin.
  • Swelling/lymphedema, which can cause skin thickening.
  • Diabetes or other metabolic conditions, depending on the appearance and location.
  • Less commonly, infections or other skin disorders.

When to seek medical care

Arrange an evaluation if the change is progressively worsening, particularly if you also have ankle/leg swelling, aching or heaviness, varicose veins, itching, or skin sores.

Seek urgent care for sudden one-sided leg swelling, warmth, redness, and pain, especially if accompanied by chest pain or shortness of breath.
