There aren’t 7 medications that are known to directly cause “severe dementia and profound brain damage” in everyone who takes them. That wording is misleading.
However, several medication classes can cause delirium, severe confusion, memory impairment, or cognitive decline, particularly in older adults or when used at high doses/for long periods. The strongest concerns include:
- Diphenhydramine (Benadryl) — strong anticholinergic effects; cumulative exposure is associated with increased dementia risk.
- Amitriptyline — strongly anticholinergic and sedating.
- Paroxetine — an SSRI with substantial anticholinergic activity.
- Oxybutynin — an antimuscarinic bladder medication; among these drugs, it has particularly strong evidence for adverse cognitive effects.
- Benzodiazepines such as diazepam, lorazepam, alprazolam, and clonazepam — increase the risk of cognitive impairment and delirium, especially in older adults.
- Zolpidem and related “Z-drugs” — can cause confusion and other adverse CNS effects, particularly in older adults.
- Antipsychotics such as haloperidol, olanzapine, and risperidone — in people who already have dementia, these drugs are associated with greater cognitive decline and increased stroke/mortality risk. PubMed Central (PMC)
Importantly, these medications do not necessarily cause permanent brain damage or dementia. Some medication-related cognitive problems are reversible after the offending drug is reduced or discontinued, while abrupt withdrawal of certain drugs (especially benzodiazepines) can itself be dangerous.
