8 Everyday Medicines That Can Affect Kidney Health — What You Should Know

The kidneys remove many medicines and their by-products from the body. Some medicines can directly affect kidney function, while others can become dangerous when kidney function is already reduced. That doesn’t mean these medicines are “bad” or shouldn’t be sold—they can provide important benefits when used appropriately. 

1. Ibuprofen and other NSAIDs

Common pain relievers such as ibuprofen, naproxen, diclofenac, and similar NSAIDs can reduce blood flow to the kidneys. The risk is greater with high doses, prolonged use, dehydration, or existing kidney disease. 

2. ACE inhibitors

Medicines such as lisinopril and ramipril can cause a small initial change in kidney filtration and, in certain circumstances, contribute to acute kidney injury. However, they’re also important kidney-protective medicines for many people with hypertension, diabetes, or kidney disease. 

3. ARBs

Drugs such as losartan and valsartan work similarly to ACE inhibitors. They can affect kidney function and potassium levels, particularly during dehydration or when combined with certain other medicines. But they can also slow kidney disease progression in appropriate patients. 

4. Diuretics

“Water pills” such as furosemide or hydrochlorothiazide help remove excess salt and water and are frequently used for high blood pressure, heart failure, and kidney disease. If the dose is too high or a person becomes dehydrated, kidney function can deteriorate. 

5. Certain antibiotics

Some antibiotics can directly affect the kidneys, while others are primarily eliminated through the kidneys and therefore may require dose adjustments when kidney function is reduced. This is why doctors and pharmacists consider kidney function when selecting and dosing certain antibiotics. 

6. Proton-pump inhibitors

Medicines such as omeprazole and similar PPIs are widely used for acid reflux and ulcers. Some research has associated PPI use with kidney problems, including acute interstitial nephritis and acute kidney injury. This doesn’t mean everyone taking a PPI will develop kidney disease.

7. Lithium

Lithium is an effective treatment for certain psychiatric conditions, but long-term treatment can affect kidney function in some people. Regular monitoring of kidney function and lithium levels is important. 

8. High-dose aspirin

Aspirin is different from low-dose aspirin prescribed for cardiovascular protection. Higher doses of aspirin, above 325 mg/day, fall into the NSAID category and can pose kidney risks, particularly in people with kidney disease. 

Why are these medicines still available?

Because risk doesn’t mean a medicine is unsafe for everyone. A medication may have substantial benefits that outweigh its potential risks when prescribed at the correct dose and monitored appropriately. Some medicines on this list can actually protect the kidneys in the right circumstances. 

One combination deserves special attention

The combination of an ACE inhibitor or ARB + a diuretic + an NSAID is sometimes called the “triple whammy.” Along with dehydration, this combination can substantially increase the risk of acute kidney injury. 

How to protect your kidneys

  • Don’t routinely take NSAID painkillers without checking whether they’re appropriate for you.
  • Tell your doctor and pharmacist about every prescription, OTC medicine, vitamin, and supplement you take.
  • If you have kidney disease, ask whether your medication doses need to be adjusted according to your eGFR.
  • Be particularly cautious with medicines during vomiting, diarrhea, fever, or dehydration.
  • Don’t stop prescribed blood-pressure or heart medicines on your own. 

Bottom line: These medicines aren’t automatically dangerous. The real issue is the right medicine, dose, combination, and monitoring for the individual patient. If you have kidney disease or are taking several medications, a pharmacist or doctor can review your complete medication list and identify potential risks.
