7 medications that cause severe dementia and profound brain damage

That headline is too strong and potentially misleading. No seven medications can accurately be described as drugs that “cause severe dementia and profound brain damage.” Some medicines, however, can cause confusion, delirium, sedation, or cognitive impairment, particularly in older adults, and some long-term exposures have been associated with increased dementia risk. Medication groups worth discussing

  • Benzodiazepines — such as diazepam, lorazepam, alprazolam, and clonazepam. In older adults they increase the risk of cognitive impairment and delirium; they also carry risks of dependence and withdrawal. PubMed Central (PMC)+1

  • First-generation antihistamines — such as diphenhydramine and doxylamine. Their strong anticholinergic effects can impair cognition, especially in older people. PubMed Central (PMC)

  • Certain tricyclic antidepressants — including amitriptyline and imipramine, which have substantial anticholinergic effects. PubMed Central (PMC)

  • Some bladder antimuscarinics — particularly oxybutynin, which has relatively strong anticholinergic activity and can adversely affect cognition. PubMed Central (PMC)

  • Certain antipsychotics — chronic use in older adults with dementia is associated with increased cognitive decline, stroke, and mortality; these medicines nevertheless have legitimate indications in selected circumstances. PubMed Central (PMC)
